Summary
Endemic goitre remains highly prevalent in Uganda at 60.2%(Burali et al., 2016) mainly due to iodine deficiency(Burali et al., 2016) and the main stay of treatment is surgery which is thyroidectomy(Fualal et al., 2012). Previously one to undertake thyroidectomy, one needed to book intensive care space since routine admission of all post thyroidectomy patients under general anaesthesia (GA) into intensive care unit (ICU) within the first 24-48 hours is advocated and practiced in most hospitals(Kolawole& Olurode., 2009). However ICU and GA come along with costs in terms of supplies, longer hospital stay etc(Mamede & Raful, 2008) & (Budhathoki et al., 2020) which most Ugandans cannot afford. Forexample on average a thyroidectomy is charged not less than 0.6million Ugandan shillings for the procedure alone(MOH/USAID;2017), but also in the error of Covid 19, GA increases inpatient Covid 19 risks. On the other hand thyroidectomy under local anaesthesia (LA) has shown to be associated with fewer post-operative complications, low costs and lesser hospital stay duration(Snyder et al., 2006),(Inabnet et al., 2008)& (Kim et al., 2017).However, no such a study has been done in a region that is highly prevalent of goitre and with few GA facilities thus generating a sound reason to comparatively profile the operation outcomes, patientโs satisfaction and cost of euthyroidectomy under GA versus LA in Uganda for local and regional comparisons.GENERAL OBJECTIVE To compare the occurrence of early postoperative complications, patientโs degree of satisfaction and the cost of euthyroidectomy under LA versus GA.SPECIFIC OBJECTIVE1. To compare the occurrence of early postoperative complications of euthyroidectomy done under LA versus GA.2. To compare the cost of euthyroidectomy done under LA versus GA.3. To compare the patientโs degree of satisfaction of euthyroidectomy done under LA versus GA.
